I got the title when i got to level 17 without getting killed one time.

Yep u are correct,
I finally got the undying title and unscathed was at 17 indeed.
I got reply from GM telling me that it was bugged and most likely the reason that i didnt get the title was because a teammate died.

I've just reached lvl 18 without dying but no title. Can anyone confirm this title actually exists. I never died but I did fail a quest to escort some NPC, who died in the proces. Maybe that's why I didn't get the title?
Anyways, I would really appreciate it if someone could tell me why I didnt get the title cuz I feel kinda screwed.
